    is holoprosencephaly compatible with life

There have been reports of sporadic cases of holoprosencephaly recognized by MRI. A few studies of limited size suggest a higher than average prevalence of HPE in Far East Asians and Filipinos. Gastrostomy tubes often are necessary to address the complex management issues of ensuring adequate calories related to feeding dysfunction and delivery of adequate free water necessary for management of diabetes insipidus. The first-trimester ultrasound can detect alobar HPE but may be much less sensitive in detecting milder cases. Often gastroesophageal reflux, choking, and gagging occur with feeds. Surgical correction of the lip to decrease the oral and nasal opening is usually delayed until the child is strong enough to tolerate the surgery and its consequences. Relatively normal facial appearance of a child with lobar holoprosencephaly resulting from a pathogenic variant in ZIC2. 2010;154C:183–90. However, for most children, no known intrauterine exposure is identified that is causally related to holoprosencephaly in that child. Holoprosencephaly is a disorder caused by the failure of the prosencephalon (the embryonic forebrain) to sufficiently divide into the double lobes of the cerebral hemispheres. This may have been due to the identification of only the most severe cases. Midline defects of the upper lip can also be attributed to holoprosencephaly (see Figure 12(c)). The most severe is cyclopia, an abnormality characterized by a single eye located in the area normally occupied by the root of the nose, and a missing nose or a proboscis (a tubular-shaped nose) located above the eye. Around 25% to 50% persons with HPE have a microscopically visible chromosomal abnormality, while 20–25% of persons with normal chromsomes have detectable defects in one or more of the genes known to be associated with HPE. Treatment and care for a baby with holoprosencephaly are supportive and based on the specific medical issues present for an individual child. In addition there are drug-inducible models that utilize small molecules which inhibit the key pathways linked to HPE (Table 35.2) (Mercier et al., 2013). Syntelencephaly (the absence or hypoplasia of the corpus callosum or absent or hypoplastic olfactory tracts and bulbs) is compatible with life and should thus be treated. The degree of motor complications (hypotonia evolving into spasticity and dystonia) and developmental dysfunction is similar to that seen in lobar HPE. In lobar HPE, approximately 50% of children ambulate (with or without assistance), use their hands functionally, and have some verbal communication. A diabetic mother's risk of having a child with HPE is approximately 1%, a greater than 100-fold increase over the general population. A small, partially formed third ventricle is seen. This agent may improve upper extremity and oromotor function. Finally, impaired horizontal cleavage manifests as a failure to form the olfactory bulbs and optic vesicles. Holoprosencephaly life expectancy A common misperception is that children with holoprosencephaly do not survive beyond early infancy. Causes, Sign, and Symptom of Holoprosencephaly: Holoprosencephaly (HPE) is the most common developmental abnormality of the forebrain in humans. The eye field begins as a single midline structure. Evidence for this malformation being a subtype of HPE is bolstered by mutation of the ZIC2 gene, which has been implicated in causing the classic forms of HPE. In: Adam MP, Ardinger HH, Pagon RA, et al., editors. 26-3). Olsen CL, Hughes JP, Youngblood LG, Sharpe-Stimac M. Epidemiology of holoprosencephaly and phenotypic characteristics of affected children: New York State, 1984-1989. In cases of Holoprosencephaly, the forebrain fails to divide into bilateral hemispheres. More common problems include slowness in eating, frequent pauses, and frank vomiting with risk of aspiration. Seattle (WA): University of Washington, Seattle; 1993-2018. About one fourth of patients have some degree of midbrain nonseparation. Excessive intestinal gas/colic, irritability, and constipation frequently occur. Hypothalamic and brain stem dysfunction may lead to swallowing difficulties and instability of temperature, heart rate, and respiration. Can you be happy living with Holoprosencephaly? Infants with cyclopia or ethmocephaly generally do not survive beyond age one week, Approximately 50% of children with alobar holoprosencephaly die before age four to five months and 20% live past the first year of life, More than 50% of children with isolated semilobar or lobar holoprosencephaly without significant malformations of other organs are alive at age 12 months. Onset of new neurologic findings or deterioration warrant evaluation for seizures and/or hydrocephalus and/or shunt malfunction. In: Woodward PJ, ed. Investigation of the epidemiology and prenatal diagnosis of holoprosencephaly in the North of England. A common misperception is that children with holoprosencephaly do not survive beyond early infancy. In nonsyndromic and nonchromosomal HPE, autosomal-dominant and autosomal-recessive pedigrees have been reported. Extra craniofacial manifestations are common. Feeding difficulties and failure to thrive may be managed with gastrostomy tube placement and Nissen fundoplication if gastroesophageal reflux and vomiting are issues. Holoprosencephaly includes a broad spectrum of defects, all based on defective formation of the forebrain (prosencephalon) and structures whose normal formation depends on influences from the forebrain. One explanation for these observations comes from studies suggesting that the mode of inheritance for HPE may be multigenic (Ming and Muenke, 2002). A. MRI in axial plane depicting lobar holoprosencephaly, the least severe of the major types of holoprosencephaly. Volpe P, Campobasso G, De Robertis V, Rembouskos G. Disorders of prosencephalic development. B. In Reference Module in Biomedical Sciences, 2014. This condition has been estimated to be present in up to 1 in 250 of all embryos and 1 in 10 000 live births. The cerebral hemispheres are separated (blue/thick arrows); the ventricles are misshapen as a result of absence of the septum pellucidum. For a general discussion of epidemiology, clinical presentation and pathology, please refer to the article on holoprosencephaly. As mentioned earlier in this chapter, the midface develops from the FNP. Patients with ZIC2 mutations can have a characteristic facial appearance (bitemporal narrowing, upslanting palpebral fissures, large ears, and a short nose with anteverted nares). Impaired transverse cleavage manifests as a failure to separate the telencephalon from diencephalon. Other treatments can be instituted as appropriate. The incidence of holoprosencephaly has been estimated at 1 in 250 during early embryonic development, and approximately 1 in 10,000–20,000 live births 5). Although relatively rare, holoprosencephaly is the most common anomaly involving the brain and the face, with an incidence of approximately 0.48–0.88 per 10 000 live-born children with normal chromosomes and an estimated 40 per 10 000 cases among human abortuses, indicating a high rate of fetal loss. The strongest teratogenic evidence exists for maternal diabetes and exposure to alcohol and retinoic acid. Cerebrospinal fluid shunts are placed in approximately 17% of survivors 30). The condition can be mild or severe. (b) The secondary vesicles at 5 weeks (fetus). Mercier S, Dubourg C, Belleguic M, Pasquier L, Loget P, Lucas J, Bendavid C, Odent S. Genetic counseling and “molecular” prenatal diagnosis of holoprosencephaly (HPE). In the most severe type, alobar HPE, nearly complete lack of separation of the cerebral hemispheres is characteristic, with a single midline ventricle very often communicating with a dorsal cyst. An updated list of genetic disorders associated with HPE can be found on the online Mendelian Inheritance in Man (OMIM) website ( Diagnostic imaging: obstetrics. As such, HPE serves as a paradigm for understanding midline signaling centers, or organizers, during forebrain development. New York: McGraw-Hill, 2001:12–16. Oral-sensory dysfunction may affect feeding especially when associated with textural aversion and labial and lingual weakness. Seattle (WA): University of Washington, Seattle; 1993-2018. TGIF1 interacts with Transforming Growth Factor (TGF) β-activated Smad proteins (Bertolino et al., 1995; Wotton et al., 1999). All kinds of holoprosencephaly can be diagnosed by ultrasound. 2012;2012:bcr0320126129. Holoprosencephaly can also occur in certain genetic syndromes in which there are other medical issues besides those mentioned in this report that affect organs in addition to the brain and face (e.g., Smith-Lemli-Opitz syndrome). Am J Obstet Gynecol 2001;184(6):1256–1262. Despite the proven usefulness of genetic mutation screening, , the exact cause of the condition is not identified for many individuals. Diabetes insipidus is particularly frequent, growth hormone deficiency, hypocortisolism, and hypothyroidism also may occur. What do you have to do to be happy with Holoprosencephaly? The gene tests, in conjunction with fetal MRI, have been found to be helpful in prenatal diagnosis and counseling in a series of pregnancies.
